Personal Outcomes

And

CCCMHMR’s Quality Assurance and Improvement System

What Is It?

Quality Improvement is a standardized system of service delivery that encourages your choice and focuses on the supports necessary to help you realize these choices. Personal outcomes are based on a set of principals which:

Promote Individual Choice

Promote Social Inclusion & Community Integration

Protects the Individual’s Health, Safety, & Welfare

The "Improvement System" puts processes in place within the organization (authority and provider systems) that support you in achieving your choices/goals and improves your quality of life.

What It Does?

It assures a high standard for safety, health, human rights, and protection from abuse.

It improves the quality of services and supports for persons served.

It enables each consumer to choose the services and supports he or she considers essential for meeting their personal goals.

It makes sure that resources are used effectively and efficiently and are specifically geared toward the consumer’s choices.

It makes providers of services responsible for improving processes and improves the quality of life for the consumer.

It creates an internal system of monitoring, separate and apart from external monitoring.

What It Means To You!

the Consumer: The ability to choose those goals that are important to you and to have supports put into place that help you reach your goals.

the Parent/Guardian: The assurance that the safety and health of your child is still protected and that the quality of life for your child is of the utmost importance to the people who work with him or her.

the Staff: Your focus will be on the choices and goals of the consumer and in the development and implementation of supports and services that help the consumer to reach his or her goals.
